Hypertension is a leading risk factor for heart disease, stroke, kidney disease, and vascular dementia in the U.S. Heart disease and stroke are the top and fifth-leading causes of death. Nearly 1 out of 2 adults in the United States has hypertension. Unfortunately, most adults with hypertension in the United States do not have their hypertension under control. More specifically, in Boston, hypertension ranged from 34% to 42% in neighborhoods like Mattapan and Dorchester, with predominantly Black populations.

At HGF, we’re looking for help to deliver our Community Blood Pressure Screenings and Health Education project this June through August. We’ve already made progress on our operational capacity and the 2HC program delivery, thanks to your previous generous support.
Our goal is to reach at least 200 residents of the targeted communities screened for hypertension and educated about measuring and controlling their blood pressure at home, at no cost at select public locations and partner events. Monitoring your blood pressure at home can help with early diagnosis and help the healthcare provider determine whether treatments are working. We will give away arm B.P. machine, B.P. monitoring card, and health and wellness educational handouts (digital and hard copies). We also plan to enroll at least 50 of them for our 2HC program, which offers 3-month RPM services, nursing visits at home, and telehealth and coaching for healthy changes.
